Once you have found advisers with whom you feel comfortable you can get them to write to the outgoing accountant seeking standard professional clearance or alternatively you should advise your previous accountant by letter or telephone call that you have changed accountants. The outgoing accountant is obliged to provide the information required by the new advisers provided professional clearance is given. It is as simple as that!
Finally, be prepared to take the risk of making the change. It will be worth it. Most accountants found to be lacking will probably be more relieved than you that you have moved on.
